Maxicare Requirements for Quotation for Companies with 100 Heads & Up
Companies with 100 heads either as combined heads of the employees or dependents or a company has 100 employees or more have a different pricing category which is called corporate. The pricing is not readily available as this has to be quoted separately. In order for us to provide you with a quotation, the company must provide the following requirements:

Maxicare Plan Types for Dependents
If you are thinking of enrolling dependents, here are the different arrangements that you can do when choosing the dependents' plan type. Note that there is a 75% participation requirement in order for the rates to be applicable otherwise an escalation fee shall be imposed.

Maxicare MaxiPlus Requirements for Enrollment
Requirements for Enrollment:
1. Filled out Maxicare Corporate Enrollment Sheet (found at the last 3 pages of this proposal). The Enrollment Sheet has 3 sheets
a. Info Sheet (Name of Company, Nature of Business, Company TIN etc)
b. Principals/Employees to be enrolled (fullnames/ birthdates/ positions/ gender/ civil status)
c. Dependents to be enrolled (fullnames/ birthdates/ positions/ gender/ civil status )
2. BIR 2303
3. Any gov’t issued ID of the signatory
4. KYC Requirements. See Pages 38-40 for the list of KYC Requirements. (to be submitted to us after we prepare the conforme for your signature).
Maxicare Rated & Ineligible Industries
Maxicare's corporate pricing can also be categorized under three (3) categories. First is the Standard Pricing for Standard Industries. Rated are for Rated Industries and Ineligible for the Ineligible Industries. In our previous proposals, we labeled the rates as Standard and Rated. For the ineligible industries, the pricing is different as this has to be customized on a per company basis.
